About The Family Model
Group Effort is proud to offer a Family Model care solution for aging adults or adults with developmental, intellectual, and physical disabilities who would benefit from living in a home environment outside of their family of origin. Think of this care option as an adult foster home. The Family Model Program is a long-term commitment from the caregiver and offers a stable home environment with 24/7 individualized care based on the needs of each person. Family Model services can be one-on-one or accommodate up to two roommates, with a maximum of three individuals receiving services in the home at any one time. The Family Model Program offers wonderful opportunities for people who want to experience independent living while still having access to the daily support they need. Group Effort is pleased to offer Family Model Care through private pay/insurance or through special programs offered by the Tennessee Department of Disability and Aging (DDA) or through the Choices and ECF Program, a Home and Community-Based Services (HCBS) initiative through Medicaid.

Group Effort conducts in-person evaluations with each family, where our team assesses the person’s individual needs and goals. Our mission is to help every adult in our program receive the fullest benefits possible. We do this by crafting personalized service plans tailored to each person, rather than placing them into preset categories.
At Group Effort, our staff promotes the highest level of person-supported independence. We strive to help them find their place in the community through employment opportunities and social engagement whenever possible. Our aim is for each person to experience a sense of fulfillment and self-worth through their unique contributions, whether that’s through work, volunteering, or simply enjoying meaningful time with family and friends.
