Whatever care option you need, one desire remains the same. The wish for your loved one to have a life filled with joy, respect, dignity and happiness and as well as a life that is as independent and fulfilling as possible. That’s our sincerest desire and goal as well because that is what every human deserves no matter their age, physical or mental disability.

In order to achieve the best results for each of our clients, Group Effort meets with each family for a face-to-face evaluation where our staff will assess the needs and goals of the individual. It is our sincerest mission to help each adult in our program to receive the maximum benefits possible. We achieve this by creating a support plan that is customized to fit the person rather than forcing the person to fit into a category.

Group Effort staff encourages the greatest possible independence for our clients. Our goal is to help them find their place in the community through employment opportunities and social interaction whenever possible, We want our clients to gain a sense of fulfillment and self-worth through their individual contributions to society in whatever way is best for them whether it be working, volunteering or just enjoying time spent with family and friends.

Family Based Care Model

Group Effort is proud to offer a Family Model care solution for elderly or disabled adults who would benefit from living in a home environment outside of their family of origin. Think of this care option as an adult foster home. The family model is a long-term commitment from the caregiver and offers a stable home environment with 24/7 individualized care based on the needs of each resident. The Family Model can be a one-on-one situation or in some cases living in a home with up to three roommates. The Family Model offers wonderful opportunities for individuals who want to experience independent living while still having access to the support services they need on a daily basis. Group Effort is pleased to be able to offer Family Based Care through private pay/insurance or through special programs offered by the Tennessee Department of Intellectual & Developmental Disabilities (DIDD) or through the Home & Community Based Services (HCBS) initiative through Medicaid. Community Based Day Support

Group Effort Day Support services are community based and are best suited for adults who require a higher level of day-to-day supervision than a family member can provide on their own. Community Based Day Support is offered outside of the individual’s home and provides opportunities to enjoy meaningful activities throughout the community. We offer this service to participants who are eligible for special help through the Tennessee Department of Intellectual & Developmental Disabilities (DIDD) or through the Home & Community Based Services (HCBS) initiative through Medicaid.

Supported Living (Shift Model)

Group Effort’s Supported Care option is best suited for individuals needing a higher level of supervision because of challenges like behavioral issues, safety risks or health related ailments. Our Supported Care service is offered in the individual’s home and provides a minimum of one staff member who is awake and on duty at all times. Additional staff members can be assigned based on the needs of the individual. This service is available through private pay or it may be covered by your insurance in some cases.

Companion Care/Personal Assistance

Group Effort’s Companion Care/Personal Assistance program is for adults residing in a family home who enjoy their independence, but need help with daily activities. This service is scheduled on an as-needed basis by the individual’s support system. We are flexible and will work with your schedule and will place you with a companion who is right for you. Whether you only need a few hours for one day, or a few hours each week, our companions are happy to help.

Group Effort’s Companions can assist with:

  • Bathing and Dressing
  • Visiting or Companionship
  • Meal Preparation
  • Light Housekeeping
  • Errands and Shopping
  • Transportation to Appointments
  • Grocery Shopping and Errands
  • Minor Home Repairs
  • Yard Work

Supported Employment Program

It’s always our goal to help our clients live their best life. Finding a job and having a purpose is often a part of that journey. Group Effort’s Supported Employment program helps to find appropriate work and provide support after our client is hired.

The first step for our clients is an assessment to discover what skill sets they may already have and to find out what, if any, barriers might need to be overcome. Next our staff will find suitable job openings to match our client’s skill sets. The support doesn’t stop after placement. Our caring staff will ensure our client’s success by assisting with training, communicating our employer partners, and even job shadowing if necessary. It’s our primary goal to see our clients find joy in the journey of working toward their life goals.

Respite Care

The Group Effort Respite Care program is geared toward adults who need temporary services because they are in crisis or are awaiting a permanent housing solution. Our respite services can provide a temporary relief for staff, family or caregivers to allow time for rest or even vacation. Group Effort respite staff may assist with daily living activities daily like bathing, dressing, hygiene, meal preparation and eating, household chores, etc. When in a client is in crisis our respite staff can help to stabilize the crisis, train the client on crisis management, help with recognizing triggers and intervention methods. Respite Care is available through private pay or in some cases it may be covered by medical insurance.
